Ozempic® is a brand-name injectable medication containing the active ingredient semaglutide. It belongs to a class of drugs known as GLP-1 receptor agonists — medications that mimic a naturally occurring hormone in your body called glucagon-like peptide-1.

Ozempic® is manufactured by Novo Nordisk, a Danish pharmaceutical company that also produces Wegovy® and several other diabetes and obesity medications. It received FDA approval in December 2017 for the treatment of type 2 diabetes in adults, where it is used alongside diet and exercise to improve blood sugar control.

Off-label prescribing is a common, legal, and well-established practice in medicine — it simply means a provider is prescribing an approved medication for a purpose other than its official FDA-approved indication, based on their professional clinical judgment and available evidence.

Ozempic® supports weight loss through three interconnected mechanisms. First, it activates GLP-1 receptors in the brain that regulate hunger and satiety signals. This reduces the constant background of hunger that many people experience, making it easier to eat less without feeling deprived.

Second, it slows the rate at which food moves through your stomach and into your intestines — a process called gastric emptying. When your stomach empties more slowly, you feel satisfied for longer after meals, which naturally reduces how much and how often you eat.

Third, it improves your body’s insulin response, which helps regulate how your body processes sugars and manages energy storage. Together, these effects create conditions where your body can more easily achieve and maintain a caloric deficit, leading to weight loss over time.

In clinical trials conducted for Ozempic®’s diabetes indication, patients experienced meaningful weight reduction as a secondary outcome. While individual results vary, many patients begin to notice changes within the first few weeks of treatment, with more significant results typically appearing over three to six months of consistent use alongside a balanced diet and regular physical activity.

Ozempic® uses a gradual dose-escalation schedule designed to minimize side effects and allow your body to adjust. Your provider will determine the exact schedule based on your clinical needs, but a typical approach looks like this:

Weeks 1–4: 0.25 mg once weekly (the initiation dose, primarily for tolerability — this dose is not expected to produce significant weight effects). Weeks 5–8: 0.5 mg once weekly. After week 8, your provider may increase to 1.0 mg once weekly. In some cases, the dose may be further increased to 2.0 mg once weekly if clinically appropriate.

Each dose is administered on the same day each week. If you need to change your injection day, you can do so as long as there are at least 48 hours between doses. Your provider will guide you through every dose adjustment and monitor your response.

Ozempic® comes in a pre-filled, multi-dose injection pen with a small, fine needle designed to minimize discomfort. You inject it under the skin (subcutaneously) in one of three recommended areas: your stomach (abdomen), the front of your thigh, or the back of your upper arm.

To use the pen: attach a new needle, dial the prescribed dose, insert the needle into the skin at a 90-degree angle, press the dose button, and hold for about 6 seconds to ensure the full dose is delivered. Rotate your injection site each week to reduce the risk of skin irritation.

While self-injection can feel intimidating at first, most patients find the process straightforward after the first one or two doses. Your provider and Care Team are available to answer questions about technique, and you can message them through the portal at any time.

Ozempic® and Wegovy® both contain the same active ingredient — semaglutide — and they work through the same GLP-1 receptor mechanism. The key differences are regulatory and clinical.

Wegovy® is specifically FDA-approved for chronic weight management in adults with obesity (BMI ≥30) or overweight (BMI ≥27) with at least one weight-related medical condition. It is available as both an injection pen and a newer oral pill formulation. Wegovy® was studied in clinical trials specifically designed to measure weight loss outcomes.

Ozempic® is FDA-approved for type 2 diabetes management. When prescribed for weight management, it is being used off-label. The dosing ranges also differ — Wegovy® goes up to 2.4 mg weekly, while Ozempic® goes up to 2.0 mg weekly.

Before starting Ozempic®, your provider needs a complete picture of your health. During your evaluation, share the following:

Your complete medical history, including any current or past conditions — particularly type 2 diabetes, pancreatitis, kidney disease, liver disease, diabetic retinopathy, or gallbladder problems. Any personal or family history of thyroid cancer, especially medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2) — Ozempic® is contraindicated in these cases.

A full list of all medications, supplements, and over-the-counter drugs you currently take — including insulin, sulfonylureas, or other diabetes medications that may interact with Ozempic®. Whether you are pregnant, planning to become pregnant, or breastfeeding — Ozempic® should not be used during pregnancy.

Any history of depression, suicidal thoughts, or mental health conditions — while rare, mood changes have been reported with GLP-1 medications and your provider should be aware. Any allergies to semaglutide or any ingredients in Ozempic®.
