The KwikPen® is a reusable pen body with a replaceable cartridge system. For Zepbound®, the pen comes pre-loaded with a tirzepatide cartridge containing multiple weekly doses at a specific concentration. You attach a new, disposable pen needle before each injection, dial your prescribed dose using the dose knob, and inject. After the injection, you remove and safely dispose of the needle, replace the pen cap, and store the pen until your next weekly dose.

Using the KwikPen® involves a few more steps than the standard single-dose pen, but the process becomes routine quickly. Here is the complete sequence:

Preparation: Wash your hands. If the pen has been refrigerated, you may let it sit at room temperature for 15-30 minutes for a more comfortable injection, though this is optional. Inspect the medication through the pen window — it should be clear and colorless. Do not use if it appears cloudy, discolored, or contains particles.

Attach a new needle: Remove the outer needle cap, screw a new pen needle onto the pen tip until snug, then remove the inner needle cap. Needles are single-use and must be replaced before each injection.

Prime the pen (first use or new cartridge only): Dial 2 units, point the pen upward, and press the injection button until a drop appears at the needle tip. This confirms the pen is functioning and removes air from the cartridge. You only need to prime once per new cartridge.

Dial your dose: Turn the dose knob until your prescribed dose appears in the dose window. Each click is an increment — listen and watch as the number increases.

Choose your injection site: Stomach (at least 2 inches from the navel), front of thigh, or back of upper arm. Clean the area. Rotate sites weekly.

Inject: Insert the needle into the skin at a 90-degree angle. Press the injection button all the way in and hold for approximately 10 seconds. The dose counter window will return to zero when the full dose has been delivered.

Remove and dispose: Pull the needle straight out. Carefully recap the outer needle cap, unscrew the needle, and dispose of it in a sharps container. Replace the pen cap and store the pen in the refrigerator.

The KwikPen® is a multi-dose, dial-a-dose pen. It holds multiple weekly doses in a single device. You attach a disposable needle before each injection, dial to your prescribed dose, inject, remove the needle, and return the pen to storage. Its strength is user control and feedback — you see your dose, hear the dial clicks, and manage the pen across multiple uses.

Neither format is objectively superior. The choice depends on what makes you more comfortable. If you want zero decisions and zero maintenance, the standard pen is likely the better fit. If you want to see and control your dose, prefer a familiar pen-style device, or have used KwikPens before for other medications, the KwikPen® may feel more natural.

The Zepbound® KwikPen® is particularly well suited for several patient profiles:

Patients with prior injectable medication experience. If you have used insulin KwikPens, Trulicity pens, or any other dial-a-dose injection device, the KwikPen® format will feel immediately familiar. You already understand needle attachment, dose dialing, and pen storage — the only new element is the tirzepatide medication itself.

Patients who want visual dose confirmation. The dose window shows exactly how many milligrams you have dialed before you inject. For patients who find reassurance in seeing the number and confirming it matches their prescription, this is a meaningful comfort factor.

Patients who prefer fewer devices. Because the KwikPen® holds multiple doses, you handle fewer individual pens over the course of your treatment. One KwikPen® may last several weeks depending on your dose and the cartridge size, compared to the standard format where each weekly dose uses an entirely new disposable pen.

Patients who are not well suited for the KwikPen® include those who are brand-new to self-injection and feel overwhelmed by the additional steps (needle attachment, dose dialing, priming). For these patients, the standard single-dose pen’s simplicity is usually the better starting point — and they can always switch to the KwikPen® later once they are comfortable with the injection process.

No. Priming is only required when you first set up a new pen or insert a new cartridge. The purpose of priming is to remove any air from the needle and cartridge assembly and confirm that medication is flowing correctly. Once the pen is primed, you do not need to repeat the process for subsequent doses from the same cartridge.

The priming process itself is simple: with a new needle attached, dial 2 units, hold the pen with the needle pointing upward, and press the injection button until a small drop of medication appears at the needle tip. This confirms the system is working and the air has been expelled. If no drop appears, repeat the priming step up to two more times. If you still do not see medication at the tip after three attempts, do not use the pen and contact your Care Team.

Even though priming is only needed at first use, you do need to attach a fresh needle before every weekly injection. Reusing needles increases the risk of infection, can cause the needle to dull (leading to more discomfort), and can affect dose accuracy.

Proper storage is important because the KwikPen® holds multiple doses across several weeks — unlike single-dose pens, which are discarded immediately after use.

Before first use, store the KwikPen® in the refrigerator at 36°F to 46°F (2°C to 8°C). Do not freeze the pen. If a pen has been frozen, do not use it — discard it and use a new one.

After first use, you have two options. You may continue to store the pen in the refrigerator between weekly doses, which is recommended. Or you may keep the pen at room temperature (up to 86°F / 30°C) for up to 21 days from the date of first use. After 21 days at room temperature, the pen must be discarded regardless of whether medication remains.

Always remove the needle after each injection and before storing the pen. Leaving a needle attached can allow air to enter the cartridge, which affects dose accuracy, and can also cause medication to leak. Replace the pen cap before storage.

Because both devices deliver the same tirzepatide at the same doses, switching between the KwikPen® and the standard single-dose pen requires no dose adjustment, no washout period, and no clinical concern. Common reasons patients switch include: discovering a preference for one device’s mechanics after trying the other, changes in availability (one format may be in stock when the other is temporarily unavailable), or lifestyle changes (a patient traveling frequently might prefer single-dose pens for convenience, then switch back to the KwikPen® at home).
